• Enter the Applicant’s Name, Date of Birth, Proposed Date of Marriage, Mother’s Name / Father’s Name.
• Under the Current Address and Permanent Address Section, Enter the House Number, Road / Street, Police Station, Post Office, G.P. / Ward, Block/S.D. Municipality, SD Municipal Corporation.
• Enter the District, State, Pin Code, and Contact Mobile Number.
• Under the Bank Account Details Section, Enter the Account Number, IFSC Code, MICR Number, Bank Name, Branch Name, and Branch Address.

• Select and tick on the Education Level, Caste, and Religion as shown below.
• Under the Groom’s Details Sections, Enter the Full Name, Date of Birth, Mother’s Name, Father’s Name.
• Under the Current Address and Permanent Address Section, Enter the House Number, Road / Street, Police Station, Post Office, G.P. / Ward, Block/S.D. Municipality, SD Municipal Corporation.
• Tick on the Declaration CheckBoxes in all the necessary checkboxes.
• Tick on any of the identity proof checkboxes you will submit along with the application form.
• Tick on the Active Bank Account Details on the application form.
• Tick on the Proof of the Proposed Marriage.

• Enter the Date and Signature of the Applicant.
• Please don’t forget to affix the applicant’s passport photo on the top of the application.
