AP Ration Card
This article explains the AP Ration Card, Online Application Procedure to Apply for the different services of the Ration Card Online on the Mee Seva Portal.
Apply AP Ration Card Online Application Forms Download 2021@ap.meeseva.gov.in
Find out the online application procedure to apply for the AP Ration Card on the official portal of Mee Seva.
- Visit the Official Portal of Mee Seva Portal.
- On the Home Page, Select and Click on the Civil Supplies Link from the set of Application Forms Section.
- Under the Civil Supply Section, Click on the Issue of New Ration Card Pink Form in English.
- It then opens up a Ration Card New Application Form, as shown below.
- In the Registration Form, Enter the Card Type, UID, Name of the Head of the Family, Father’s Name, Mother’s Name, in English and Local Language.
- Select Gender, Spouse Name, Date of Birth and Age.
- In the Professional Details Section, Enter the Occupation and Annual Income.
- In the Gas Connection Details, Enter Gas Connection Status, Company Name, Agency Name and Consumer Number.
- In the Residence Section, Enter the Door Number, Locality, State, District, Mandal, Village/Ward, Pin Code and FB Shop Number.
- Also, Enter the details of the Permanent Address.
Conversion of Ration Card from White to Pink Application PDF Form@ap.meeseva.gov.in
- On the Home Page, under the Civil Supplies Section, Click on the Conversion of White Card to Pink Card Form.
- It then opens a pdf application form to convert the white ration card to a pink ration card.
- Enter the Ration Card Number in the Request Details Section.
- In the Service Specific Details, Card Type, New Income, Reasons for Changing Card Type.
- In the Informant Details Section, Enter the Informant Name, Relation, Mobile Number, and Delivery Type.
- In the Documents List Section, Carry the Filled Application Form, Copy of Old Ration Card, and Income Certificate.
- Keep the Applicant Signature at the end of the page.
Ration Card Member Addition(Birth) Application Form
- Under the Civil Supplies Section, Click on the Ration Card Member Add (Birth) Application Form on the Home Page.
- It then opens a pdf application form of the Ration Card Member Addition Form.
- Enter the Ration Card Number in the Request Details Section.
- In the Member Addition Section, Enter Member Name, Mother Name, Date of Birth, Gender, Age, Father Name, Spouse Name, EID, UID and Relationship with Head.
- In the information details section, Enter Applicant Name/House Head Name, Informant Name, Informant Relation, Mobile Number, Delivery Type, Proof Document.
- Find out the Documents List to be attested on the bottom of the form.
- Sign at the bottom after filling the form.
Ration Card Member Migration Application Form PDF
- Under the Civil Supplies Section, Click on the Ration Card Member Migration Application Form on the Home Page.
- It then opens up the pdf form, as shown below.
- Enter the Migration From Ration Card Number and Migration to Ration Card number in the Request Details Section.
- Enter Migration Member Details like Name, Date of Birth, Relation with Head and Active Status.
- Enter Informant Name, Informant Relation, Mobile Number, Delivery Type, Proof Document and find out the necessary documents to attach.
- At the bottom of the page, the Applicant has to give his signature.
Deletion of Member in Ration Card Application
- Under the Civil Supplies Section, Click on the Deletion of Member in Ration Card Application Form on the Home Page.
- In the Request Details Section, Enter the Ration Card Number.
- In the Member Deletion Details Section, Enter Name, Date of Birth, Relation and Action.
- In the Informant Details Section, Enter the Informant Name, Reason, Relation, Mobile Number and Delivery type.
- Find out the list of the documents to be uploaded in the documents section.
- At the bottom of the page, the Applicant has to give his signature.
AP Ration Card Transfer Application Form
- Under the Civil Supplies Section, Click on the Deletion of Member in Ration Card Application Form on the Home Page.
- In the Request Details Section, Enter the Ration Card Number.
- In the Card Transfer Details Section, Enter the House Number to be Changed, Land Mark/Locality/Colony to be Changed, District to be Changed, Mandal, Ward/Village and Pin Code to be Changed.
- Tick Mark on the two documents you are submitting as proof for a card transfer.
- In the Informant Details Section, Enter the Informant Name, Reason, Relation, Mobile Number, Delivery type, Reason for Card Transfer, and Old Ration Card collected from the Applicant.
- Find out the list of the documents to be uploaded in the documents section.
- At the bottom of the page, the Applicant has to give his signature.
Surrender of Ration Card Application Form PDF
- Under the Civil Supplies Section, Click on the Surrender of Ration Card Application Form on the Home Page.
- In the Request Details Section, Enter the Ration Card Number.
- Choose the Reason for Surrender.
- In the Informant Details Section, Enter the Informant Name, Reason, Relation, Mobile Number, Delivery type and Old RC collected from the Applicant.
- Find out the list of the documents to be uploaded in the documents section.
- At the bottom of the page, the Applicant has to give his signature.
